Introduction
State Initiative on Disabilities (SID) of Kerala Social Security Mission deals with disability aspects and its management. The objective of SID is to initiate and implement programmes for the prevention, early screening, early intervention, rehabilitation, education and employment of persons with disabilities. State Initiative on Disabilities implement programme with the support of Social Justice Department, Women and Child Department, Health Department, Education Department and through reputed institutions like National Institute of Speech and Hearing (NISH), National Institute of Physical Medicine and Rehabilitation (NIPMR), Institute of Cognitive and Neuro Sciences (ICONS), Government Medical Colleges, Kerala State Handicapped Persons Welfare Corporation (KSHPWC) etc. The programmes of State Initiative on Disabilities are implemented using the budgetary allocation of State Government.
2. Universal Hearing Screening at Birth:
With the objective of early screening and early intervention of hearing disability, hearing screening of neonates has commenced. At present, the hearing screening of new born are done in 61 Government Delivery points in the State. Oto Acoustic Emission Screeners (OAE) are provided to these delivery points along with the service of one trained JPHN for doing the screening. The referred cases are followed up by the JPHNs. Under this programme, around 120,000 newborns were screened.
3. Confirmation of hearing disability through BERA test:
By three months’ time through advanced tests the hearing problem can be confirmed. For this one BERA equipment each is installed in all the 14 districts in the National Programme for Prevention and Control of Deafness centers at the District/ General Hospitals.
4. Auditory Verbal Therapy Centre at Govt. Medical College, Kozhikode and Trivandrum and National Institute of Physical Medicine and Rehabilitation (NIPMR), Kallettumkara, Thrissur :
It is mandatory that children who are identified for cochlear implant surgery should undergo pre implant auditory verbal therapy and post implant auditory verbal habitation therapy for a minimum period of two to three years regularly after surgery. High quality services need to be ensured in both situations for better outcome. With the objective of enabling children with hearing impairment to learn, listen, and understand language and to communicate with speech through scientific technologies and appropriate procedures and equipment, SID has established State-of Art Auditory Verbal Therapy Centres (AVT) at ENT Department of Govt. Medical College Kozhikkode& Trivandrum and National Institute of Physical Medicine and Rehabilitation (NIPMR),Kallettumkara, Thrissur.
5. Regional Early Intervention Centers (REICs):
REICs are established as apex institutions for disability management. At present REICs are established in 5 Medical College Hoospitals- Thiruvananthapuram, Alappuzha, Kottayam, Trissur and Kozhikode Medical College Hospitals. REICs are providing better quality services through convergence with different disciplines of Medical Colleges. Service of One Pediatrician or One Medical Officer –MBBS, One Physiotherapist, One Occupational Therapist, One Audiologist & Speech Therapist, One Clinical Psychologist, One Developmental Therapist, One Special Educator, One Optometrist, One Staff Nurse, One Social Worker, One Data Entry Operator and REIC Manager is available in each REIC for providing intervention, treatment and remedial therapies to children with disabilities. Every year around 25000 children with disabilities were receiving services of REIC.
6. Mobile Intervention Units (MIUs):
The objective of the project is to provide regular services and therapy to children with disabilities residing in rural areas who abstain from such services due to various reasons. 25 MIUs established across the State. One MIU caters six block panchayats. Each MIU consists of one Developmental Therapist, one Physiotherapist, one Special Educator, one Clinical Psychologist and one Speech Therapist cum Audiologist. The project is implemented jointly with National Health Mission.
7. Model Child Rehabilitation Centers (MCRC):
Providing interventional services through Child Rehabilitation Centers are the accepted protocols in disability management of children. Building construction of MCRCs in Mattannur Municipality is completed.Equipment has been provided for MCRCs at Madayi and Erinjoli Panchayats of Kannur District and Thanalur Panchayats of Malappuram District.
8. Special Anganwadi Project:
To provide appropriate training and remedial therapy to pre-school children with disabilities especially developmental disabilities Special Anganwadi Project started in Kozhikode District. Under this Project, one Anganwadi in each ICDS Project is designated as nodal Special Anganwadi with 2 satellite special anganwadies in the same ICDS project. One Special Educator with degree or diploma in Special Education has been deployed for each nodal Special Anganwadi for providing appropriate training and remedial therapy to children with disabilities. Other Anganwadi Workers will also be trained for identifying pre school children with development delay and other disabilities and for giving basic training in remedial therapy. 2248 children with disabilities were provided with special care through the special Anganwadies and 1141 children among them were capacitated to join to regular school for inclusive primary education.

9. Help Desk:
A centralized Help Desk is functioning at the office of KSSM at Poojappura with a toll free number- 1800-120-1001 for providing information and support services to Persons with Disabilities (PwDs) regarding the various schemes, concessions, programmes and also to support them to avail these services. Everyday around 150 persons with disabilities are availing services through Centralized Help Desk.
10. Autism Centres:
For screening and for providing interventional services to Children with Autism Spectrum Disorder, 7 Autism Centres were established in 6 Govt Medical Colleges- Thiruvananthapuram, Alappuzha, Kottayam, Thrissur, Kozhikode and Manjeri and one in IMHANS, Kozhikode. Services of Audiologist cum Speech Therapist, Development Therapist, Occupational Therapist, Physiotherapist, Special Educator and Social Worker are available in these centers. Every month around 2500-2600children with Autism Spectrum Disorders are getting services out of these centers.
11. Regional Autism Research Centre at National Institute of Physical Medicine and Rehabilitation (NIPMR), Kallettumkara, Trissur:
With the objective of providing services to autistic children and their families using modern facilities in diagnosis, assessment, evidence-based personalized appropriate interventions, development of personalized and structured care plan, continuous assessment of co-morbidities, family education and support, empowerment of family, monitoring and management, etc, Regional Autism and Research Centre is established in NIPMR, Thrissur. The centre is conducting training programmes, research and studies on ASD and organize training programmes for Professionals. Services of Pediatrician, Social Workers, Clinical psychologist, Development Therapist, Speech Therapists, Occupational Therapists, Physiotherapist, Special Educators, Dietician, part time Teachers for Music/ Dance are available in the centre.
12. Child Development and Research Centre
Child Development and Research Center with modern facilities is established at the National Institute of Physical Medicine and Rehabilitation, Thrissur, Irinjalakuda, to provide special rehabilitation activities through modern and scientific interventions to children under the age of three who are suspected of autism, and to conduct studies and research in the field of autism.
13. Cerebral Palsy Rehabilitation and Research Center
Cerebral Palsy Rehabilitation and Research Center has been set up at the National Institute of Physical Medicine & Rehabilitation, Iringalakuda, to carry out rehabilitation activities for children with cerebral palsy with modern facilities, as well as research activities and studies in this field. These centers provide therapies and training with the help of modern equipment.
14. Project for Empowering Persons with disabilities through Horticulture Therapy and Social Farming:
Horticulture Therapy is a new area of intervention in disability management. It is a therapy using plants and horticultural activities to improve the social, educational, psychological and physical adjustments of persons to improve their body, mind and spirit. This particularly help children of special needs to adapt, cope, develop and expand their personal abilities and potentials. The role of horticulture activities and the use of natural environment by way of horticulture therapy has been suggested as a rehabilitative tool by physicians and psychologists. This can be done by developing specially designed gardens which can be managed by children with special needs. SID jointly with Kerala Agricultural University, established a Horticulture Therapy Garden at College of Agriculture, Vellayani, Thiruvananthapuram and at Padanna, Kasargode.
15. Project to prevent disability due to Ankylosing Spondylitis.
In order to prevent possible disabilities due to Ankylosing Spondylitis, a scheme is being implemented to provide free treatment to deserving patients. A state registry for ankylosing spondylitis patients has also been developed. The scheme is being implemented through the nodal centers at Govt Medical Colleges at Thiruvananthapuram and Thrissur and free medicines are made available to the patients.
16. A special plan to prevent potential disability due to Multiple Sclerosis
As part of prevention of disability due to Multiple Sclerosis, the scheme is being implemented to provide free medicine and treatment to the economically backward patients. State registry for Multiple Sclerosis patients has been developed. Sree Chitra Thirunal Hospital, Thiruvananthapuram and Government Medical College, Kozhikode have been identified as nodal centers to provide medicines to the patients.
17. Project for people with severe nervous system disorders
As part of empowering services for people suffering from various difficulties and disabilities due to severe disorders of the nervous system, a state-of-the-art Gait and motion Analysis Lab has been established at the National Institute of Physical Medicine & Rehabilitation, Irinjalakuda. Also, in the Spinal Injury Rehabilitation Center operating in this center, the necessary equipment has been installed viz. ceiling lift, secondary complication assessment monitoring systems, empowerment of patient support services, etc. required for this category of patients.
18. Rehab Express
State Initiative on Disabilities - Rehab Express is implemented in association with NIPMR to provide interventions including therapies to the persons with disabilities and to assist the local self-governing bodies in activities including assessment and training in the individual disability assistive devices, distribution schemes, by the local self-government institutions / bodies.
19. Dream Dare Do:
SID had produced 4 Short Films on persons with disabilities who overcome disabilities and become achievers, for motivating children with disabilities and their parents through social media screening and screening in special schools, institutions for the disabled, public functions organized by Government Departments and agencies, academic and training institutions. These motivational short films are based on the life and achievements of Ms. Swapna Augustine (Foot Artist), Mr. Anjan Sathish (Caricaturist with multiple disability), Ms. Tifani Brar (Blind, Social Activist) and Mr. Vishnu (Intellectually Challenged Person with Cerebral Palsy, Anuyatra Ambassador).
20. District Early Intervention Centers (DEICs):
SID has taken steps for establishing full fledged permanent District Early Intervention Centers with all facilities for ensuring early screening of disabilities and for providing appropriate services. The construction of DEIC Kollam, Thrissur, Malappuram, Kannur and Wayanad are completed and handed over to National Health Mission.
